Medical

The employee benefit market continues to evolve with the implementation of the Affordable Care Act and Shore continues to respond keeping patients and employees' needs first.  Shore is proud to provide eligible employees with an expanded menu of (5) benefit plan options.  These options  include a 100% benefit for the SMC facility,  discounted copays for Shore Quality Partner (SQP) providers participating in the Horizon Network, in addition to local Shore Urgent Care facilities.   All plan choices are open access encompassing the national Blue Card network, requiring  NO REFERRALS which allow the members and their families the freedom of shopping their benefit options and self-directed care.

Vision

Each medical plan is inclusive of a vision benefit of one well exam per year within the network.   Shore also offers VSP Vision Care via our voluntary benefit options. 

Prescription

RX is administerd by Express Scripts prescription plan.  Prescriptions for up to 30 days may be despensed at any retail pharmacy, with lower out of pocket cost if filled at Shore Medical Center's employee pharmacy.  Members are encouraged to fill 90 day and maintenance drugs at the Shore employee pharmacy at a reduced rate or at a Smart 90 Pharmacy.  Shore's employee pharmacy is participating in the Smart 90 pharmacy network.

Dental

Shore offers three dental plans.  Most plans cover preventive, basic, restorative and major services inclusive of orthodontia.  Shore gives their members the option of buying up to High Option or a Total Care plan and/or adding dependents.

Basic and High Option Dental

Basic Dental Option is free to employee (only) contracts.  Employees who select the Horizon Dental Option Plans (Basic or High Option) are not required to select a Primary Care Denist but they must utilize a Horizon Traditional Dentist for an In-Network level benefit.

Total Care

Subsribers must select a Primary Care Dentist from the Horizon TotalCare Network.  Services are available only when provided or coordinated by a Horizon TotalCare Dental Dentist.

Supplmental Short-Term Disability

STD can give you the financial support you need until you are back on your feet and back to work.

Critical Illness & Cancer

Covers many out-of-pocket expenses associated with critical illness and pays a lump sum cash benefits to the insured upon diagnosis of a covered critical illness.

Legal Services

Legal Insurance provides access to legal support when needed.

Accident

Personal Accident Insurance will help pay medical and other out-of-pocket expenses incurred when suffering an accidental injury.

Vision 

VSP Vision will provide for vision exams outside your medical plan as well as, the purchase of frames, contacts, and covered vision surgery at a discounted price.

Pet Insurance

Pet Insurance protects your pet and your pocketbook against unexpected veternarian costs.

Life Insurance

Shore Medical Center provides all regular employee scheduled 24 hours a week or more access to employer paid Basic Life and Accidental Death and Dismemberment (AD&D) insurance.  In addition, employees have the option of supplementing their employer sponored plans with the following voluntary benefit options for themselves and dependents.

  • Supplemental employee Life Insurance - 1x - 3x annualy salary
  • Supplemental spouse Life  - 50% of employees annual salary
  • Dependent Child/ren Life
401K

Shore helps their employees prepare for retirement with a robust 401(k) Savings and Retirement Plan.  You are automatically enrolled at a 2% employee contribution unless you choose to either opt-out or increase your contributions.  Shore matches 50% of your contribution each pay up to 4% depending on your job category.  Please contact Human Resources for information on job categories yeilding higher contribution match.
